2002-2003 Immunization Entry Requirements for Schools, Preschools and Child Care Facilities
Get charts and immunization forms for the 2002-2003 school year
• Proof of three doses of Hepatitis B
• Proof of four DTP (Diphtheria, Tetanus,
Pertussis) with the last dose given on or after the fourth birthday
• Proof of four doses of Hib
• Proof of three polio with the last dose given on or after the fourth birthday
• Proof of two MMR (Measles, Mumps, Rubella) given on or after the first birthday and at least 28 days apart
Requirements for Fifth Graders Prior to Sixth-grade Entry:
• Proof of a second MMR vaccine
This is also the time to update adolescents (11-12 years old) if they need:
• A TD booster (provided it's been at least five years since their last tetanus/diphtheria containing dose of vaccine) now required on or before 17 years of age.
